When attempting to change a student’s language assignment from a non-English language to English, an HLS Amendment Request is required to be completed and sent to IDOE for review prior to a change occurring. Due to the time needed for review and the volume of requests at the beginning of a school year, HLS Amendment Requests must be submitted by Friday, September 18, to be reviewed prior to the October 1 Multilingual Learner Report count date. Due to FERPA, HLS Amendment Request forms along with the student’s original HLS must be submitted via Jotform.
