To streamline processes and improve efficiency, all HLS amendment requests must be submitted via Jotform beginning Monday, March 30. Previously, amendments were submitted via fax to IDOE. Any amendments submitted via fax after this date may experience delays, as the fax inbox will no longer be monitored.
