To streamline processes and improve efficiency, all HLS amendment requests must be submitted via Jotform effective Monday, March 30. Previously, amendments were submitted to IDOE via fax. Any amendments submitted via fax after this date may experience delays, as the fax inbox will no longer be monitored.
